CVS Health is seeking a remote RN Case Manager to work from home in the United States. The role involves telephonic case management for fully and self-insured clients, applying clinical guidelines and coordinating with care teams.

Required are 5+ years as an RN with multistate licensure (NLC) and experience with electronic documentation and oncology. Strong communication and motivational interviewing skills are essential.

Qualifications

• 5+ years' experience as a Registered Nurse, including at least 1 year in a hospital setting.
• RN must have an active multistate license through NLC or state-specific licenses for each state supported.
• 1+ years' experience documenting electronically using a keyboard.
• 1+ years' experience in Oncology.

Responsibilities

• Serve as a telephonic case manager for patients and care teams.
• Apply criteria and clinical guidelines to determine benefits and needs.
• Use case management to reduce risk factors and address health barriers.
• Assess information from multiple sources to address co-morbid conditions.
• Consult with supervisors and multidisciplinary teams to meet goals.
