Job summary

Wellmark is seeking a Case Management Nurse I to actively engage members through various channels, build individualized care plans, and coordinate services with providers and families. You will support chronic and complex health needs while maintaining high standards of documentation and regulatory compliance.

All qualified candidates must complete an accredited nursing program, hold an RN license in IA or SD, and obtain CCM within 24 months.

Qualifications

• Completion of an accredited nursing program.
• Active, unrestricted RN license in Iowa or South Dakota (reside in state licensed).
• Certified Case Manager (CCM); must obtain within 24 months of hire.

Responsibilities

• Utilize critical thinking to assess high-risk/complex conditions and coordinate care.
• Discuss care needs with members and plan interventions.
• Identify barriers to care and design individualized care plans.
• Coordinate with members, families, providers and community resources.
• Educate members on benefits and responsibilities within their plan.
• Document activities accurately in the case management system and follow guidelines.
• Perform other duties as assigned.
