Job summary

Wellmark, Inc. is seeking a Case Management Nurse I to support members with chronic and complex health needs. You will develop individualized care plans, coordinate with providers and resources, and guide members through care pathways in a remote-capable role.

The position requires an accredited nursing program completion, an active RN license in IA or SD, and CCM certification within 24 months. A background in diverse clinical settings and strong communication skills are essential.

Qualifications

• Completion of an accredited nursing program.
• Active and unrestricted RN License in Iowa or South Dakota.
• Certified Case Manager (CCM); must obtain within 24 months of hire.
• 4+ years of diverse clinical experience reflecting direct care to consumers.
• Strong verbal communication skills; influences action and facilitates conversations.
• Strong written communication skills with accurate documentation.
• Knowledge of URAC, NCQA, HIPAA and PHI confidentiality.
• Ability to manage multiple priorities in a dynamic work environment.

Responsibilities

• Utilize critical thinking to recognize signs of potential high‑risk and complex conditions and anticipate needs.
• Discuss care needs with the member through supportive interventions and planning.
• Identify barriers and design individualized care plans to ensure effective communication and appropriate care.
• Partner with members, families, providers and community resources to coordinate and facilitate care.
• Educate on included benefits and services relevant to current status and outcomes.
• Collaborate with the Wellmark team and stakeholders to provide optimal service.
• Document care management activities accurately and timely per guidelines and policies.
• Remain compliant with regulatory and accreditation standards; perform other duties as assigned.
