Job summary

Elevance Health is seeking a Nurse Case Manager I to manage care for members with complex needs. You will assess, develop, implement, coordinate, monitor, and evaluate care plans, with telephonic or on-site duties such as discharge planning.

Candidates should have a BA/BS in a health-related field, 3 years of clinical experience, and an active RN license with multi-state eligibility. The role offers hybrid/virtual work with required in-person training and a comprehensive benefits package.

Qualifications

• Requires BA/BS in a health-related field and minimum of 3 years of clinical experience.
• Current, unrestricted RN license in applicable state(s). Multi-state licensure required if providing services in multiple states.

Responsibilities

• Performs care management within licensure scope for members with complex and chronic needs.
• Assess, develop, implement, coordinate, monitor, and evaluate care plans.
• Coordinate authorizations/referrals and resources to meet identified needs.
• Monitor effectiveness of care plans and modify as necessary.
• Interface with Medical Directors and Physician Advisors on care plans.
• Assist in problem solving with providers, claims or service issues.
