Job summary

National Care Advisors seeks a Nurse Case Manager based in Portland to oversee case management for special needs and eldercare, coordinating with families, attorneys, financial planners, and trustees to improve quality of life. The role requires strong communication, organization, and ability to adapt to a virtual work environment.

Preferred candidates hold a BSN and CCM/CRRN; 5+ years of clinical nursing and case management experience, with travel as needed.

Qualifications

• BSN required; CCM or CRRN preferred.
• Minimum 5 years clinical nursing experience; field or workers’ compensation case management preferred.
• Experience with third-party benefits: health insurance, Medicaid, Social Security, Developmental Disability services.
• Excellent written and verbal communication skills.
• Proficient in using MacOS and MS Office.
• Ability to learn new technologies in a Mac environment.
• Self-starter with strong time management and organizational skills in a virtual setting.
• Ability to collaborate to achieve goals and resolve conflicts.
• Ability to research and develop client solutions.
• Excellent customer service skills.
• Overnight travel regionally 1-2 times per month and occasional nationwide travel.
• Willingness to meet timely documentation requirements.

Responsibilities

• Provide case management for special needs and eldercare clients and quality of life.
• Collaborate with primary family caregivers, attorneys, financial planners and trustees.
• Coordinate documentation and reporting to ensure compliance and timely updates.
• Support clients and families through care planning and advocacy.
