Job summary

National Care Advisors seeks a Nurse Case Manager based in Eau Claire, WI to coordinate eldercare, special needs case management and quality of life planning, collaborating with families, attorneys, financial planners and trustees.

You will work in a flexible, remote-friendly environment with a salaried schedule, required travel regionally and some nationwide travel as needed, and a strong emphasis on timely documentation and customer service.

Qualifications

• BSN is required; CCM or CRRN preferred.
• Minimum of 5 years clinical nursing experience with case management preferred.
• Experience with third-party benefits including health insurance, Medicaid, and Social Security services.

Responsibilities

• Coordinate case management services for eldercare and special needs.
• Collaborate with primary family caregivers, attorneys, financial planners, and trustees to support client goals.
• Meet documentation requirements in a timely manner and support quality-of-life improvements.
