DispatchHealth seeks a Virtual Nurse Coordinator to join the Hospital at Home program. The RN delivers coordinated virtual and in-home acute care using clinical judgment, technology, and evidence-based practice to support safe, effective care at home.

Collaborating with interdisciplinary teams, you will assess patient needs, address social determinants of health, educate patients and families, and ensure smooth transitions of care across states. Remote role with per diem flexibility.

Qualifications

• Registered Nurse with at least 3 years of experience in acute care, medical-surgical, emergency, home health, geriatrics, or care coordination.
• Current BLS/CPR certification required on hire and maintained throughout employment.
• State licensure as a Registered Nurse and ability to obtain additional licensure as needed for remote care across multiple states.

Responsabilités

• Assess and monitor patient condition using clinical judgment and critical thinking; escalate when needed.
• Coordinate care across interdisciplinary teams to ensure seamless transitions and timely interventions.
• Advocate for patients and families, promoting shared decision-making and education throughout the care journey.
• Collaborate with patients, caregivers, providers, and community resources to implement individualized care plans.
• Utilize telehealth technologies, remote monitoring, and EHR systems to support safe care delivery.
• Maintain accurate clinical documentation per regulatory and organizational standards.
• Foster safe care transitions and address social determinants of health impacting outcomes.
