Job summary

VCU Health is seeking a Home Hospital Registered Nurse to deliver inpatient care within patients' homes using telehealth and remote coordination. You will apply the nursing process in a team model, document care, and work with a multidisciplinary team to ensure high-quality outcomes.

Responsibilities include in-home and remote assessments, education, care planning, and support to reduce readmissions. The role requires travel to off-site locations and collaboration with the virtual nursing team

Qualifications

• Minimum of three years of critical care, acute care, or medical-surgical RN experience.
• Current Virginia RN license or eligibility/compact.
• BSN preferred or enrolled in BSN program.
• AHA BLS/HCP certification or equivalent required.
• Commitment to diversity, equity and inclusion in care delivery.

Responsibilities

• Provide nursing care remotely or in home visits using the nursing process.
• Collaborate with the 24/7 virtual nursing team and providers.
• Triage, identify changes in patient condition and respond promptly.
• Coordinate resources for Home Hospital patients and coordinate services.
• Educate patients and families on technology use and care plans.
• Travel to off-site locations as needed to support patients.
