Job summary

Trinity Health At Home is seeking a Virtual Care Coordinator RN to deliver telehealth hospice care, monitor patient conditions remotely and coordinate comprehensive care plans with a multidisciplinary team.

This role requires 5+ years in home care/hospice, an active compact-state RN license, and BSN. You will be part of a remote, patient-centered care team with opportunities for professional growth.

Qualifications

• RN with 5+ years in home care, hospice, ER, or triage
• Active RN license in a Compact State (eNLC)
• BSN required
• Experience with Epic or Home Care EMR
• Strong virtual communication skills
• Ability to work independently in a remote team
• Patient-focused and adaptable

Responsibilities

• Conduct virtual assessments and monitor patients remotely.
• Develop and coordinate individualized care plans with the care team.
• Provide virtual education and support to patients, families, and caregivers on symptom management and end-of-life care.
• Liaise with field nurses, physicians, social workers, and others to coordinate care.
• Respond promptly to urgent virtual consults and triage needs.
• Use telehealth to perform regular check-ins and follow-ups.
