Job summary

Honest Health is seeking a Nurse Care Manager to support patients with complex chronic conditions through virtual care coordination. You will partner with physicians, APPs, and care teams to develop individualized nursing care plans that align with patient values and goals.

Remote role based in the United States, requires Nevada RN licensure and 3+ years of clinical experience. Proficiency with EHRs and Microsoft Office is expected; licensed in NV with willingness to obtain additional state

Qualifications

• Active RN license in Nevada is required.
• 3+ years of clinical practice preferred.
• Experience with case management and chronic disease care.
• Experience with electronic health records (EHR) required.
• Microsoft Office skills including Excel, Word, PowerPoint, and Outlook.

Responsibilities

• Manage patients with complex needs in collaboration with physicians and care team.
• Develop comprehensive nursing care plans aligned with patient goals.
• Coordinate care across hospitals, SNFs, home care, and vendors.
• Support remote patient engagement and education along with providers.
