Job summary

Honest Health is seeking a Nurse Care Manager to remotely manage patients with complex needs, collaborating with physicians and care teams to develop and implement comprehensive care plans. You’ll coordinate services, monitor outcomes, and advocate for patients across the Honest network.

You must hold an active RN license in New York or Michigan, have 3+ years of clinical practice, and be comfortable with virtual patient engagement and EHR use.

Qualifications

• Active Registered Nurse license in the state(s) where the role is based: Michigan and/or New York.
• 3+ years of clinical practice in a hospital, clinic, home care, or nursing home setting highly preferred.
• Experience and high level of comfort with engaging patients virtually using different technologies including phone, text, video, etc. is required.
• Must have reliable access to high-speed internet to ensure seamless remote work communication and productivity.
• PowerPoint experience preferred.

Responsibilities

• Build empathetic relationships with patients and families.
• Evaluate and identify patients’ needs based on values, goals, and preferences; translate into clinical needs.
• Interface with physicians, APPs, specialists, and disciplines on care plans.
• Coordinate access to health services across providers and disciplines.
• Refer patients to Honest team members for care management activities as outlined by procedures.
• Monitor care and identify cost-effective measures; suggest changes in levels of care as needed.
• Coordinate admissions and discharges from hospitals or SNFs with clinicians and case managers.
• Build relationships with network hospitals, SNFs, home health, and DME vendors.
• Evaluate the effectiveness of care plans and modify as needed.
• Coordinate continuity of care including transfers and authorizations for outside services.
• Educate patients to manage their condition and participate in their plan of care.
• Deliver disease-specific education to patients and families.
• Address social determinants of health in care planning.
• Collect and analyze data on clinical outcomes and customer satisfaction; support action strategies.
• Participate in huddles, IDT sessions, and patient case conferences.
