  • Conduct a clinical intake via email or phone call to assess the client's needs and determine the appropriate step forward in their healthcare journey.
  • Collaborate closely with client services coordinators, medical records, and provide network team to holistically manage cases to provide the appropriate provider recommendations, coordinate appointments, assist with medical record review and support, and provide clinical guidance when necessary
  • Provide cross collaboration support to our triage team who handles inbounds via email and phone calls for clients with active needs or who are interested in our services
  • Communicate clearly and compassionately with clients, caregivers, providers, and internal partners.
