Qualifications

• DNP, FNP, ENP, or PA from accredited program.
• 3+ years experience in outpatient Family Medicine, Internal Medicine, or Primary Care.
• 2+ years of experience delivering virtual care.

Responsibilities

• Provide high-quality care for patients in a virtual setting.
• Use prescriptive authority as part of a collaborative practice with physicians on the care team for comprehensive care, including complex patients.
• Utilize tools to perform remote exams, home monitoring, and remote diagnostics.
• Participate in a collaborative care model with nurses, medical assistants, PCPs, behavioral health clinicians, and clinical supervisor where applicable.
• Attend meetings such as huddles, case conferences and All Hands to support patient care.
