Job summary

Local Infusion is hiring a Nurse Practitioner to provide telehealth care for neurology and memory care patients. You will assess patients, craft care plans, manage treatments with physician oversight, and educate families on diagnoses and progression.

The role is remote, requires MA licensure with geriatric specialization preferred, and involves collaborating with neurologists to ensure seamless care and proper documentation in the EMR.

Qualifications

• Active, unrestricted NP license in Massachusetts; geriatrics/adult-gerontology/family practice certifications preferred.
• 2+ years of clinical experience in neurology, memory care, or dementia care settings.
• Strong clinical judgement and robust documentation skills.
• Ability to manage a full patient panel independently with physician collaboration.
• Excellent communication with patients and caregivers facing cognitive decline.

Responsibilities

• Provide direct clinical care to neurology/memory care patients via telehealth.
• Assess patients, build care plans, and manage ongoing treatment with supervising physicians.
• Collaborate with neurologists and care team to ensure continuity of care.
• Educate patients and families on diagnoses, treatment plans, and progression.
• Maintain accurate EMR documentation in compliance with regulations.
