Job summary

Local Infusion is seeking a Nurse Practitioner for a remote, patient-facing role focused on neurology and memory care. You will deliver telehealth care to patients, perform assessments, and collaborate with supervising physicians to optimize treatment plans.

Qualifications

• Active, unrestricted NP license in Maine.
• 2+ years in neurology, memory care, or dementia care.
• Certification in geriatrics, adult-gerontology, or family practice preferred.
• Strong clinical judgement and accurate documentation.
• Ability to manage a full patient panel with physician collaboration.

Responsibilities

• Provide direct telehealth clinical care for neurological and memory disorders.
• Assess patients, develop care plans, and manage treatments with supervising physicians.
• Collaborate with neurology team to ensure continuity of care.
• Educate patients and families on diagnoses and progression.
• Maintain accurate EMR documentation in compliance with regulations.
