Local Infusion is seeking a licensed Nurse Practitioner in Connecticut to join our remote care team, delivering virtual neurological and memory care services to patients. You will conduct telehealth assessments, develop care plans, and prescribe therapies in collaboration with supervising physicians, while educating families and maintaining EMR documentation to regulatory standards.

Qualifications

• Active, unrestricted NP license in Connecticut.
• Certification in geriatrics, adult-gerontology, or family practice preferred.
• 2+ years of clinical experience in neurology, memory care, or dementia care settings.
• Strong clinical judgment and clinical documentation skills.
• Comfortable managing a full patient panel with physician collaboration.
• Excellent communication with patients facing cognitive decline and caregivers.

Responsibilities

• Provide direct clinical care to patients with neurological conditions via telehealth.
• Conduct patient assessments, develop care plans, and manage ongoing treatment with supervising physicians.
• Prescribe therapies in collaboration with physicians.
• Partner with neurologists and care team to ensure continuity of care.
• Educate patients and families on diagnoses, treatment plans, and disease progression.
• Maintain accurate, timely clinical documentation in the EMR.
