Job summary

Local Infusion is seeking a licensed Nurse Practitioner in Connecticut to join our virtual care team in a remote, patient-facing role focused on neurology and memory care. You will collaborate with physicians to deliver telehealth care to patients, manage treatment plans, and ensure timely access to therapy.

You will conduct assessments, prescribe therapies in coordination with supervising physicians, educate families, maintain EMR documentation, and contribute to a supportive, growth-oriented

Qualifications

• Must hold active unrestricted NP license in CT.
• Minimum 2 years clinical experience in neurology/memory care.
• Telehealth experience is a plus.
• Excellent documentation and communication skills.

Responsibilities

• Provide direct clinical care to patients via telehealth.
• Conduct patient assessments and develop care plans with supervising physicians.
• Prescribe therapies in collaboration with physicians.
• Educate patients and families on diagnoses and treatment plans.
