Local Infusion is seeking a Nurse Practitioner licensed in Connecticut to join our remote, patient-facing care team. You will deliver telehealth services for neurological and memory care patients, partnering closely with physicians to increase access and shorten time to treatment.

Responsibilities include patient assessments, care plan development, independent panel management with physician collaboration, and educating patients and families.

Qualifications

• Active, unrestricted NP license in Connecticut; geriatrics/adult-gero/family certification preferred.
• 2+ years of clinical experience in neurology, memory care, or dementia care settings.
• Strong clinical judgement and accurate documentation.
• Ability to manage a full patient panel with physician collaboration.
• Excellent communication with patients with cognitive decline and caregivers.

Responsabilités

• Provide direct clinical care to patients with neurological conditions via telehealth.
• Assess patients, develop care plans, and manage treatment with supervising physicians.
• Partner with neurologists and care team to ensure continuity of care.
• Educate patients and families on diagnoses, treatment plans, and progression.
• Maintain accurate EMR documentation in line with regulatory requirements.
