Zusammenfassung

Local Infusion is seeking a skilled Nurse Practitioner licensed in Connecticut to join our remote care team. This is a patient-facing telehealth role delivering care to neurology and memory care patients, partnering with physicians to improve access and shorten time to treatment.

You will assess patients, develop care plans, manage ongoing treatment, and prescribe therapies in collaboration with supervising physicians.

Qualifikationen

• Active, unrestricted NP license in Connecticut.
• Certification in geriatrics, adult-gerontology, or family practice preferred.
• 2+ years of clinical experience in neurology, memory care, and/or dementia care.
• Strong clinical judgement and clinical documentation skills.
• Comfortable managing a full patient panel independently with physician collaboration.
• Excellent communication skills with patients facing cognitive decline and their caregivers.

Aufgaben

• Provide direct clinical care to patients with neurological conditions via telehealth.
• Conduct patient assessments, develop care plans, and manage ongoing treatment in collaboration with supervising physicians including prescribing therapies.
• Partner with our neurologist(s) and broader care team to ensure continuity of care.
• Educate patients and families on diagnoses, treatment plans, and disease progression.
• Maintain accurate, timely clinical documentation in the EMR.
