Local Infusion is seeking a Nurse Practitioner (CT licensed) to join our remote care team, delivering telehealth neurology and memory care services to patients across our infusion program.

You will assess patients, develop care plans, prescribe therapies in collaboration with supervising physicians, educate families, and maintain accurate EMR documentation while ensuring high-quality patient access and timely treatment.

Qualifications

• Active, unrestricted NP license in Connecticut.
• Certification in geriatrics, adult-gerontology, or family practice preferred.
• 2+ years of clinical experience in neurology, memory care, and/or dementia care.

Responsabilités

• Provide direct clinical care to patients with neurological conditions via telehealth.
• Assess patients, develop care plans, and manage treatment in collaboration with supervising physicians.
• Educate patients and families on diagnoses, treatment plans.
• Maintain accurate clinical documentation in the EMR in line with regulatory requirements.
