Job summary

My Local Infusion is seeking a Nurse Practitioner licensed in New Hampshire to join our virtual care team. This remote, patient-facing role delivers care to patients with neurological conditions and memory disorders, partnering with physicians to improve access and treatment timelines.

You will conduct assessments, develop care plans, manage treatments, document in the EMR, and educate patients and families. Remote work supports flexibility and work-life balance.

Qualifications

• Active, unrestricted NP license in New Hampshire.
• 2+ years of clinical experience in neurology, memory care, and/or dementia care settings.
• Certification in geriatrics, adult-gerontology, or family practice preferred.

Responsibilities

• Provide direct clinical care to patients with neurological conditions, memory disorders, and/or dementia via telehealth.
• Conduct patient assessments, develop care plans, and manage ongoing treatment in collaboration with supervising physicians including prescribing therapies.
• Partner with our neurologist(s) and broader care team to ensure continuity of care.
• Educate patients and families on diagnoses, treatment plans, and disease progression.
• Maintain accurate, timely clinical documentation in the EMR consistent with regulatory requirements.
