Job summary

Local Infusion seeks a licensed Nurse Practitioner in New Hampshire to join our remote-first care team. This is a patient-facing telehealth role delivering neurology and memory care services, partnering with physicians to improve access and time to treatment.

The successful candidate will provide direct clinical care, assess patients, develop care plans, prescribe therapies in collaboration with supervising physicians, educate families, and maintain compliant EMR documentation.

Qualifications

• Active, unrestricted NP license in New Hampshire.
• Certification in geriatrics, adult-gerontology, or family practice preferred.
• 2+ years of clinical experience in neurology, memory care, and/or dementia care settings.
• Strong clinical judgment and clinical documentation skills.
• Comfortable managing a full patient panel independently with physician collaboration.

Responsibilities

• Provide direct clinical care to patients with neurological conditions via telehealth.
• Conduct patient assessments, develop care plans, and manage ongoing treatment with supervising physicians including prescribing therapies.
• Partner with neurologists and care team to ensure continuity of care.
• Educate patients and families on diagnoses, treatment plans, and disease progression.
• Maintain accurate, timely clinical documentation in the EMR in line with regulatory requirements.
