Job summary

Local Infusion is seeking a licensed Nurse Practitioner in Virginia for a remote, patient-facing neurology and memory care role. You will deliver telehealth care, develop care plans, and collaborate with supervising physicians to improve access and speed to treatment.

Ideal candidates have 2+ years in neurology/memory care, strong documentation skills, and the ability to manage a patient panel with physician support. Join a remote-first team focused on growth and patient-centered care.

Qualifications

• Active, unrestricted NP license in Virginia.
• Certification in geriatrics, adult-gerontology, or family practice preferred.
• 2+ years of clinical experience in neurology, memory care, and/or dementia care settings.
• Strong clinical judgement and clinical documentation skills.
• Comfortable managing a full patient panel independently with physician collaboration.

Responsibilities

• Provide direct clinical care to patients with neurological conditions via telehealth.
• Conduct patient assessments, develop care plans, and manage ongoing treatment in collaboration with supervising physicians including prescribing therapies.
• Partner with our neurologist(s) and broader care team to ensure continuity of care.
• Educate patients and families on diagnoses, treatment plans, and disease progression.
• Maintain accurate, timely clinical documentation in the EMR consistent with regulatory requirements.
