Job summary

Liberty Healthcare is hiring for a Level of Needs Assessor to support a time-limited project starting mid-October and lasting roughly two to five months. The role is remote, with occasional in-person meetings in the local region to engage with individuals and families.

Administer LON assessments, complete forms within deadlines, and coordinate with guardians and caregivers to tailor supports for people with developmental disabilities. Idaho residency is required.

Qualifications

• Bachelor's degree in a human service-related field.
• At least one year of experience supporting people who are living with autism, developmental disabilities or traumatic brain injury.
• Self-motivated, organized, and able to work independently with minimal supervision.

Responsibilities

• Receive training on the Level of Needs (LON) assessment.
• Administer LON assessments to individuals who are living with developmental disabilities.
• Meet primarily by video and occasionally in-person with people and their families, guardians and caregivers.
• Complete structured forms and reports within established timeframes.
