Job summary

Equity Staffing Group seeks a Clinical Pharmacist residing in Oklahoma with an active Oklahoma Pharmacist license to support MTM and clinical programs remotely. The role focuses on medication therapy management, adherence, diabetes care, polypharmacy, and retrospective drug utilization, with telephonic consultations and comprehensive reviews.

Responsibilities include identifying gaps in care, communicating recommendations to providers, and maintaining HIPAA-compliant documentation.

Qualifications

• Active Oklahoma Pharmacist license.
• Must reside in Oklahoma and be eligible for remote MTM work.
• 1+ year of remote/independent experience in a cross-functional environment.
• Strong knowledge of MTM and the 5 Core Elements of MTM Services.
• Proficient with Microsoft Word, Outlook, and Excel.

Responsibilities

• Conduct telephonic consultations and comprehensive medication reviews with members.
• Review medications to identify gaps in care, adherence concerns and opportunities.
• Assess regimens for effectiveness, dosing, and adverse reactions.
• Provide patient education on medications and disease-state management.
• Communicate clinical recommendations to members and providers.
• Document patient care, interventions, and recommendations.
• Maintain HIPAA compliance and meet productivity and quality expectations.
