Job summary

Equity Staffing Group seeks a Clinical Pharmacist who resides in Alabama to join a Clinical Engagement Services Center supporting MTM, diabetes management, adherence, and other clinical programs. Remote role with Alabama residency required.

The role requires MTM proficiency, strong communication, and ability to work independently in cross-functional teams from home; extended hours may be needed.

Qualifications

• Active Alabama Pharmacist license
• 1+ year of remote work experience
• Strong knowledge of MTM and disease-state management

Responsibilities

• Conduct telephonic consultations and comprehensive med reviews
• Review medications for gaps in care, adherence, interactions, high-risk meds
• Assess regimens for effectiveness, dosing, adverse reactions
• Provide patient education on meds and disease-state management
• Communicate recommendations to members and providers
• Document patient care, interventions, and recommendations
• Maintain HIPAA compliance and meet productivity/quality targets
