Job summary

Equity Staffing Group is seeking a Clinical Pharmacist based in Tennessee to support a Clinical Engagement Services Center focused on MTM and medication management. The role is remote, requiring residency in Tennessee, with duties including telephonic reviews and cross-functional collaboration.

The ideal candidate will hold an active Tennessee Pharmacist license and have MTM experience, strong communication, and the ability to manage multiple MTM platforms while documenting care accurately.

Qualifications

• Residence in Tennessee is required.
• Active and unrestricted Tennessee Pharmacist license.
• Knowledge of the 5 Core Elements of MTM Services.
• 1+ year remote/independent experience in a cross-functional environment.
• Ability to navigate MTM platforms and multiple applications.
• Strong communication, documentation, organization, and time-management skills.
• Proficiency with Microsoft Word, Outlook, and Excel.

Responsibilities

• Conduct telephonic consultations and comprehensive medication reviews with members.
• Review medications and clinical information to identify gaps in care, adherence concerns, etc.
• Assess medication regimens for effectiveness, appropriate dosing, and potential adverse reactions.
• Provide patient education on medications and disease-state management.
• Communicate clinical recommendations to members and providers.
• Accurately document patient care, interventions, and recommendations.
• Maintain HIPAA compliance and meet productivity and quality expectations.
