Job summary

Pharmacy Careers is seeking an MTM Pharmacist to join our growing Medication Therapy Management team, often from home or with flexible hybrid schedules. You will conduct comprehensive medication reviews, coach patients on adherence, and improve outcomes—sometimes from home with flexible hybrid options.

Eligible candidates hold a PharmD or Bachelor of Pharmacy, with ambulatory, hospital, or community MTM experience encouraged.

Qualifications

• PharmD or Bachelor of Pharmacy education required.
• Ambulatory, hospital, community MTM experience encouraged.
• Licensed pharmacist applicants welcome, including new graduates.

Responsibilities

• Perform comprehensive medication reviews (CMRs) and targeted reviews with patients.
• Identify medication-related problems, interactions, adherence barriers, and cost-saving opportunities.
• Collaborate with prescribers and care teams to resolve therapy issues and document interventions.
• Educate patients and caregivers on safe, effective medication use.
• Maintain CMS MTM program compliance and quality metrics.
