Job summary

Pharmacy Careers is seeking an MTM Pharmacist to join our growing team. Conduct comprehensive medication reviews, coach patients on adherence, and improve outcomes—often from home or with flexible hybrid schedules.

Qualifications

Responsibilities

• Perform Comprehensive Medication Reviews (CMRs) and Targeted Medication Reviews with patients.
• Identify medication-related problems, drug interactions, adherence barriers, and cost-saving opportunities.
• Collaborate with prescribers and care teams to resolve therapy issues and document interventions.
• Educate patients and caregivers on safe, effective medication use.
• Maintain CMS MTM program compliance and quality metrics.
