Job summary

Pharmacy Careers is seeking an MTM Pharmacist to help patients optimize medications, with remote options and flexible scheduling. You will conduct comprehensive medication reviews, coach patients on adherence, and collaborate with prescribers to improve outcomes.

Qualifications

• PharmD or Bachelor of Pharmacy required or equivalent.
• Ambulatory, hospital or community MTM experience is a plus.
• All licensed pharmacists, including new graduates, are encouraged to apply.

Responsibilities

• Perform comprehensive medication reviews (CMRs) and targeted reviews with patients.
• Identify medication-related problems, interactions, adherence barriers, and cost savings.
• Collaborate with prescribers and care teams to resolve therapy issues and document interventions.
• Educate patients and caregivers on safe, effective medication use.
• Maintain CMS MTM program compliance and quality metrics.
