Job summary

Pharmacy Careers is seeking MTM Pharmacists to conduct comprehensive medication reviews and optimize therapy for patients, often from home or with flexible hybrid schedules.

You'll coach patients on adherence, collaborate with care teams, and help reduce hospitalizations through proactive medication management in ambulatory settings.

Qualifications

Key Responsibilities

What You’ll Bring

• Education: PharmD or Bachelor of Pharmacy.
• Experience: Ambulatory, hospital, community, or prior MTM experience is a plus. All licensed pharmacists, including new graduates, are encouraged to apply.
• Skills: Excellent patient counseling, independent work style, and clear documentation.
