Job summary

Pharmacy Careers is seeking a licensed pharmacist for an MTM Pharmacist role with remote or hybrid options. You will conduct Comprehensive Medication Reviews, coach patients on adherence, and work with care teams to optimize therapy in ambulatory settings.

Ideal candidates hold a PharmD or Bachelor of Pharmacy, with ambulatory, hospital, or community MTM exposure, and a strong ability to document interventions clearly. This position supports flexible schedules and meaningful clinical impact.

Qualifications

• Pharmacists with MTM experience or related ambulatory care exposure are preferred.
• Strong documentation of interventions and clear patient communication is required.
• Ability to work independently and manage time effectively.

Responsibilities

• Perform Comprehensive Medication Reviews (CMRs) and Targeted Medication Reviews with patients.
• Identify medication-related problems and opportunities for cost savings.
• Collaborate with prescribers and care teams to resolve therapy issues and document interventions.
• Educate patients and caregivers on safe, effective medication use.
• Maintain CMS MTM program compliance and quality metrics.
