Job summary

Spring Health is seeking an independently licensed mental health professional to join our Care Navigation team, supporting members with mood and anxiety disorders. This full-time, remote role involves initial assessment, coaching, referrals, and ongoing care coordination with Spring Health providers.

You’ll apply motivational interviewing, DBT-informed coaching, and evidence-based care planning, while collaborating with cross-disciplinary teams to ensure a high-touch member experience.

Qualifications

• Required mental health license: LCSW, LMFT, LPC.
• Minimum of 2+ years of clinical care experience working with chronic and severe mood and anxiety disorders.
• Experience with motivational interviewing, diagnostic assessment, and triage.
• Knowledge of current best practices and evidence-based treatments for mood and anxiety disorders, with a strong preference for DBT.
• Excellent listening, written and verbal communication skills.
• Acknowledge and embrace diversity and inclusion; experience working with diverse populations.
• Motivated to go above and beyond for members to provide a high-touch experience.
• Tech-savvy: comfortable using various computer platforms and learning new systems; able to multi-task and context switch.

Responsibilities

• Telephonic screening, brief intervention, and referral of members to a mood and/or anxiety treatment provider.
• Apply active listening, psychoeducation, and motivational interviewing to understand members' needs and goals.
• Utilize DBT skills to coach members as part of their care journey.
• Coordinate care with a Spring Health provider and identify appropriate levels of care.
• Provide consultation on mental health concerns and family issues as appropriate.
• Provide in-the-moment support and safety planning for crisis situations.
• Document information and maintain accurate records in an integrated EHR system.
• Interact with multi-disciplinary teams to ensure a smooth member experience; assist with onboarding and mentoring new team members.
