Job summary

Spring Health in the United States is hiring independently licensed mental health professionals to join the Care Navigation team. LCSW, LMFT, LPC, or licensed psychologist required.

The role includes in-the-moment clinical support, crisis assessment, safety planning, and coordinating care across therapists, coaches, physicians, and external resources. Shift is Monday night/Tuesday morning to Friday night/Saturday morning, ET, with potential holidays.

Qualifications

• Licensed mental health professional with independent licensure.
• Minimum of 2+ years of direct clinical care experience.
• Crisis assessment and safety planning expertise.
• Strong communication and ability to work independently.

Responsibilities

• Provide in-the-moment clinical support via clinical phone queue.
• Use active listening and motivational interviewing to engage members.
• Assess risk, provide crisis support, and safety planning.
• Coordinate plan of action and connect to Spring Health providers or external resources.
• Provide after-hours support and interactions with multi-disciplinary teams.
• Complete professional documentation and communicate via email.
• Follow outreach protocols and identify barriers to care.
• Collaborate with care providers and operations teams for seamless care.
• Participate in staff meetings, case consultations, and trainings.
• Support onboarding and mentoring of new team members.
