Qualifications

Responsibilities

• Contact high-risk members and providers to close care gaps.
• Support quality initiatives by reaching out to members who have not completed services.
• Conduct seasonal outreach campaigns in coordination with local markets.
• Enter assessments and authorizations into designated systems and maintain databases.
• Maintain confidentiality and integrity of personal health information.
• Follow procedures and perform other related duties.
