Job summary

Ascendion is seeking an experienced Medical Coder to join our remote team. You will review clinical documentation, assign ICD-10-CM diagnosis codes and CPT procedure codes, and ensure coding accuracy and compliance for outpatient encounters.

Ideal candidates have 2–4 years of medical coding experience, CPC/CCS certification, and strong knowledge of medical necessity and E/M coding. This role offers remote flexibility and comprehensive benefits.

Qualifications

• 2–4 years of medical coding experience required.
• Hands-on experience with ICD-10-CM and CPT coding.
• Experience with outpatient or physician professional-fee coding.
• Active CPC, CCS, or equivalent certification required.
• Knowledge of E/M coding, medical necessity, and clinical documentation review.

Responsibilities

• Review clinical documentation and assign ICD-10-CM diagnosis codes and CPT procedure codes.
• Code for primary care and outpatient physician encounters.
• Review medical records for coding accuracy, completeness, and compliance.
• Validate codes against documentation and guidelines.
• Evaluate E/M coding and medical necessity for appropriateness.
• Identify discrepancies, gaps, and potential compliance issues.
• Support coding audits, quality reviews, and validation activities.
• Maintain accurate coding practices per industry standards.
