Job summary

Qualifications

• Medical Assistant or equivalent certification is required.
• Minimum 3 years of experience in a medical setting (primary care, geriatrics, home health/hospice).
• Experience with electronic medical records (EMR).

Responsibilities

• Make outgoing patient calls after hospital discharge to meet regulatory requirements.
• Contact patients, families, and caregivers per clinician instruction.
• Coordinate with social workers and community resources to support home-based care.
