Job summary

UnitedHealthcare is seeking a Maternal Registered Nurse Case Manager in Washington. You will liaise with Medicaid members who are pregnant or recently pregnant to ensure access to appropriate care, perform home and social assessments, and educate members and families.

You will coordinate care with internal teams, providers, and community resources, develop person-centered care plans, promote self-management, and support discharge planning.

Qualifications

• Current, unrestricted RN license in Washington
• 2+ years of maternal clinical experience
• 1+ years of experience with MS Office (Word, Excel, Outlook, Teams)
• Reside in the state of Washington and have a designated quiet workspace

Responsibilities

• Engage members telephonically to complete comprehensive assessments across medical, behavioral, functional, cultural and socioeconomic needs
• Develop and implement person-centered care plans addressing chronic conditions, wellness, social determinants, and safety
• Collaborate with internal care teams, providers and community resources to execute care plans
• Provide education and coaching to support member self-management and healthy lifestyle changes
• Support proactive discharge planning and manage care transitions
• Advocate for members and families to ensure their needs are represented by the care team
