• 1+ year of experience with MS Office (Word, Excel, Outlook, Teams).

Responsibilities

• Engage members telephonically to complete a comprehensive needs assessment.
• Develop and implement person-centered care plans addressing health needs.
• Partner with internal care teams, providers, and community resources.
• Provide education and coaching to support member self-management.
• Support proactive discharge planning and manage care transitions.
• Advocate for members and families to ensure their needs are represented.
