Job summary

UnitedHealth Group's UnitedHealthcare division seeks a Maternal Registered Nurse Case Manager in Washington state. The role coordinates care for pregnant or recently pregnant Medicaid members, performing home and social assessments and educating members and families.

You will work with internal care teams, providers, and community resources to develop person-centered care plans, manage chronic conditions, and promote health and safety. Remote work in the Greater Seattle area is supported.

Qualifications

• Current, unrestricted RN licensure in Washington state.
• 2+ years of maternal clinical experience.
• 1+ years of experience with MS Office (Word/Excel/Outlook/Teams).

Responsibilities

• Engage members telephonically for needs assessments across medical, behavioral, and social domains.
• Develop and implement person-centered care plans addressing chronic conditions and social determinants.
• Collaborate with care teams, providers, and community resources to execute care plans.
