Job summary

UnitedHealthcare is seeking a Maternal Registered Nurse Case Manager to coordinate care for pregnant and recently pregnant Medicaid members in Washington. This role includes home and social assessments, member education, and partnering with the care team to improve outcomes, with a focus on social determinants of health and safe transitions.

Remote work is available for residents of Washington state in the Greater Seattle/Tacoma area.

Qualifications

• Current, unrestricted RN licensure in the state of Washington.
• 2+ years of Maternal clinical experience.
• 1+ years of experience with MS Office (Word, Excel, Outlook, Teams).
• Designated, quiet workspace with access to secure high-speed internet at home.
• Reside in the state of Washington.

Responsibilities

• Engage members telephonically to complete comprehensive needs assessments.
• Develop and implement person-centered care plans addressing medical, behavioral, and socioeconomic needs.
• Collaborate with internal care teams and community resources to implement care plans.
• Provide education and coaching to support self-management.
• Support discharge planning and coordinate care transitions.
• Advocate for members and families to ensure needs are represented.
