Job summary

UnitedHealthcare is seeking a Maternal Registered Nurse Case Manager to coordinate care for Medicaid members who are pregnant or recently pregnant, including home and social assessments and education. This role supports medical, behavioral, and socioeconomic needs and offers remote work for Washington residents in the Greater Seattle/Tacoma area.

Responsibilities include comprehensive telephonic assessments, care planning, collaboration with care teams, and advocacy to ensure optimal service

Qualifications

• Current, unrestricted RN licensure in the state of Washington.
• 2+ years of Maternal clinical experience.
• 1+ years of experience with MS Office, including Word, Excel, Outlook, and TEAMS.

Responsibilities

• Engage members telephonically to complete a comprehensive needs assessment covering medical, behavioral, functional, cultural, and socioeconomic needs.
• Develop and implement person-centered care plans addressing chronic conditions, health promotion, social determinants, and safety.
• Collaborate with internal care teams, providers, and community resources to implement care plans.
• Provide education and coaching to support member self-management of care needs and lifestyle changes.
• Support proactive discharge planning and coordinate care transitions.
• Advocate for members and families to ensure their needs are represented by the care team.
