Job summary

UnitedHealthcare is seeking a Maternal Registered Nurse Case Manager in Washington state. The role focuses on coordinating care for pregnant or recently pregnant Medicaid members, conducting home and social assessments, and providing education to members and families.

Responsibilities include telephonic needs assessments, care plan development, collaboration with care teams, and supporting transitions while advocating for members’ choices. Remote work within WA is possible.

Qualifications

• Current, unrestricted RN licensure in the state of Washington.
• 2+ years of Maternal clinical experience.
• 1+ years of MS Office experience (Word, Excel, Outlook, Teams).
• Reside in the state of Washington.
• Preferred: CCM certification; NICU/Pediatric exposure; Managed Care; Bilingual English/Spanish.

Responsibilities

• Engage members telephonically to complete comprehensive needs assessments across medical, behavioral, functional, cultural, and socioeconomic needs.
• Develop and implement person-centered care plans for chronic conditions, health promotion, social determinants of health, medication management and safety.
• Collaborate with internal care teams, providers, and community resources to implement care plans.
• Educate and coach members to support self-management and healthy lifestyle changes.
• Support discharge planning and coordinate care transitions.
• Advocate for members and families to ensure needs and preferences are respected by the care team.
