Job summary

BLEHEALTH seeks a Lead Care Manager (LVN) to coordinate with chronically ill members, families, providers, and community resources. The role oversees ECM services, implements care plans, and advocates for members to access timely care.

This remote position requires a personal vehicle, valid license, and occasional office visits. Mileage reimbursement is provided. Starting pay ranges from $30 to $32 per hour, depending on experience and shift.

Qualifications

• Associate’s or bachelor’s degree in health science preferred.
• LVN or Social Worker background with case management experience a plus.
• Own a vehicle with valid driver’s license and insurance; travel to homes/hospital visits as needed.
• Must present Negative TB test and CPR certification before hire date.
• Live Scan/background check required.

Responsibilities

• Coordinate with members, families, and providers to ensure seamless care and avoid service duplication.
• Engage eligible members and oversee ECM services and care plan implementation.
• Promote timely access to care and reduce ER/hospital readmissions.
• Provide health promotion and self-management training to members.
• Educate members and families about community resources and transportation options.
• Accompany members to visits and coordinate transitions-in-care with hospitals and PCPs.
• Maintain regular contact with care team via face-to-face, phone, email, or secure messaging.
