Job summary

Chronic Care Staffing LLC is seeking a CA-licensed LVN to serve as a Remote Patient Care Coordinator. This full-time, 100% remote role focuses on proactive patient outreach, care coordination, and documentation in the EHR from your home office.

You will review symptoms, medications, and care needs, escalating concerns as needed while maintaining strong communication with patients and healthcare teams. A CA LVN license and current BLS are required.

Qualifications

• Active, unrestricted California LVN license.
• Current BLS certification.
• Recent nursing experience in primary care, chronic disease management, or high-volume outpatient setting.
• Strong clinical judgment and patient communication skills.

Responsibilities

• Make proactive outbound patient calls.
• Review symptoms, medications, chronic conditions, and care needs.
• Recognize changes in patient status and escalate concerns appropriately.
• Reinforce patient education and care plans within your scope of practice.
• Coordinate with providers, pharmacies, and other members of the healthcare team.
• Document patient interactions accurately and efficiently in the EHR.
• Manage patient follow-ups and ongoing care needs.
• Meet quality and productivity expectations, including at least 2 billable patient encounters per hour.
