A leading care management organization is seeking a Clinical Lead Care Manager to be the main point of contact for clients in need of complex care. The role involves assessing client needs, coordinating with multiple providers, and advocating for client services. Candidates must possess an active California LVN license, have 2+ years of related experience, and be bilingual in Spanish/English. This hybrid position requires travel within Santa Clara County, making residency within 25 miles of the area necessary.

Formación

• 2+ years of experience as a care manager or related role.
• Willing to work Monday-Friday from 8:30am-5:00pm.
• Clean driving record and reliable transportation.

Responsabilidades

• Assess member needs across various health dimensions.
• Oversee development of client care plans and goals.
• Advocate for clients with healthcare professionals.
• Conduct outreach to facilitate program linkage.
