Job summary

BLEHEALTH seeks a Lead Care Manager (LVN) to collaborate with chronically ill members, families, providers, and community resources. You will oversee ECM services, implement care plans, advocate for members, and promote self-management and timely access to care.

The role requires driving to hospitals and home visits, with remote work options and periodic satellite office meetings. An LVN or social work/case mgmt background is preferred.

Qualifications

• An associate’s degree, or bachelor's degree in health science or any related health care degree is preferred
• Social Worker, LVN, or experience in case management is a PLUS!

Responsibilities

• Coordinate with members and care teams to ensure seamless service and avoid duplication
• Engage eligible members and oversee ECM services and care plan implementation
• Provide care where member lives/seeks care within guidelines and connect to social services
• Advocate for members with healthcare professionals and coordinate with hospitals on discharge plans
• Monitor medication adherence and promote preventative care
• Educate members and families with culturally/linguistically appropriate information
• Promote self-management and shared decision making
• Arrange transportation and follow up with members, families, providers, and resources
