Geisinger is seeking a Director of Service Coordination and Long-Term Services and Supports (LTSS) to lead strategy, program design, and implementation for all LTSS/HCBS requirements under the Community HealthChoices agreement in Pennsylvania. This role supports CHC delivery while enabling work-from-home arrangements within the state and occasional travel across PA.

• Master's Degree required: Healthcare, Nursing, Social Work, Public Administration, or related field.
• Minimum 8 years managerial/supervisory experience.
• Minimum 5 years Health Insurance/Managed Care experience.

Responsabilidades

• Lead strategy and implementation for CHC LTSS programs.
• Develop and oversee partnerships with LTSS providers and community groups.
• Represent GHP in DHS/OLTL forums and prepare readiness reports.
• Oversee budgets and ensure CHC funding aligns with program goals.
• Ensure compliance with state/federal regulations and contract obligations.
