Geisinger is seeking a Director for Service Coordination and LTSS to lead strategy, program design, and execution for the Community HealthChoices agreement in Pennsylvania. The role involves building partnerships, guiding policy implementation, and ensuring high-quality LTSS delivery across the state.

The successful candidate will work with Geisinger Health Plan leadership, travel across Pennsylvania, and oversee audits, reporting, and contract compliance while promoting a participant-centered

Formación

• Master's Degree in Healthcare, Nursing, Social Work, Public Administration, or related field.
• Minimum of 8 years of managerial/supervisory experience.
• Minimum of 5 years in Health Insurance/Managed Care.

Responsabilidades

• Supports strategy and implementation of CHC program, including Service Coordination and Service Integration.
• Develops and oversees strategic partnerships with LTSS providers, community organizations, advocacy groups, and vendors.
• Represents GHP as a CHC leader and LTSS SME in state forums and audits.
• Prepares for CHC readiness reviews and program standards audits.
• Monitors and reviews contract-required reporting and signs off as needed.
• Develops policies to ensure participant-centered LTSS delivery.
• Collaborates with leadership to address operational issues and resource needs.
