Qualifications

• Requires 1–2 years of related experience.
• Preferred 2+ years supporting LTSS for adults.
• Experience with outbound member/provider outreach.
• Experience processing healthcare/insurance authorizations and coordinating services.

Responsibilities

• Provide outreach to members via phone to support care plan steps and scheduling.
• Connect members to health plan and community resources to ensure quality care.
• Serve as frontline support for member/provider inquiries and explain care plan procedures.
• Document and maintain non-clinical member records per state regulations and provide to providers as needed.
• Assist onboarding and distribute welcome letters and program materials to facilitate relationships.
• Identify and refer to SDOH resources to address recipient needs.
